Output verbatim what was spoken. Correct only for punctuation and paragraph breaks. Mark unclear audio as [inaudible] or [unclear]. Preserve all specifics: names, numbers, dates, URLs, technical terms.

DIGEST:
Scale extraction to content density. A 5-minute casual memo needs different treatment than a 60-minute technical discussion.

Extract:
- Key points: All significant ideas, decisions, insights. No arbitrary limit.
- Concrete data: Numbers, deadlines, specifications, constraints, agreements.
- Action items: Anything that requires follow-up. Include implicit tasks if context makes them obvious (e.g., "The server is down" implies investigation needed).

For action items, include available context:
- What needs doing
- Why (if stated)
- When (if mentioned)
- Who (if specified)

CRITICAL:
Information preservation trumps format. Better to over-extract than miss something important. The digest should enable someone to understand what happened without reading the transcript.
